Do you suffer from Sleep Apnea?


Don’t worry, you’re not alone.
A lot a people suffer from Sleep apnea and don’t even know it!
Check out this article written by Dr Christopher Chang by Fauquir ENT, where he explains why people suffer from this and how they can treat it.

Sleep endoscopy is a special procedure to determine exactly where an upper airway obstruction is localized causing obstructive sleep apnea or snoring while a person is sleeping.

There are two distinct components to this procedure:

1) Endoscopy is when a thin flexible camera is inserted through the nose in order to examine the entire upper airway from the tip of the nose to the voice box.
2) Sleep (or sedated) endoscopy is when the endoscopy is performed while a patient is sleeping instead of wide-awake as traditionally performed.

Given most patients will immediately wake up if a doctor is inserting something into the nose, an anesthesiologist uses IV medications to sedate the patient.
What are some of these areas of obstruction?
Areas that will be specifically examined during sleep endoscopy include:
Behind the palate
Uvula
Back of Tongue
Walls of the Throat
Epiglottis
Voice box

Ever wondered about your Tonsils?


I know I have… Well check out this article written by Dr. Christopher Chang from Fauquir ENT
Where he explains the importance of your Tonsils and what happens when they are enlarged or swollen.

 The tonsils are located in the back of the mouth flanking the uvula along the sides of the throat. The adenoids are located above and behind the uvula and can only be seen on endoscopy. The tonsils and adenoids are basically lymph nodes just like in your neck, but rather than being hidden under the skin, they are on the surface of your throat where you can see them. As such, removal of the tonsils and adenoids have minimal, impact on a patient’s immune system as there are thousands of other lymph nodes that can pick up the slack.

The tonsils and adenoids generally regress in size with age to the point they can be quite small in adults. However, it is not unusual for children to have enormous tonsils the size of walnuts and even ping pong balls. Adenoids may be similarly enlarged.

Sometimes the tonsils can be so large that a patient may have trouble swallowing and even affecting taste such that eating becomes a chore. Not only that, they may impact sleep as there is not much room for air to pass resulting in obstructive sleep apnea which is a serious medical condition.

Symptoms of obstructive sleep apnea include:
loud snoring
restless sleep
daytime sleepiness
irritable daytime behavior
difficulty waking
witnessed apneas and/or struggling to breath at night
head bent backwards while sleeping
mouth-breathing while sleeping

Sinus problem? We’ve got you covered with a Balloon!


That’s right! There is a NEW technic that Doctors use that helps with sinus problems.
Check out this article written by Dr. Christopher Chang from FAUquier ENT, where he explains this amazing procedure.
In a human being, there are 4 sinus cavities that come in pairs (1: frontal, 2: ethmoid, 3: sphenoid, 4: maxillary). Each of these sinus cavities drain through a small opening into the nose. However, problems occur when the openings clog shut resulting in recurrent and even chronic sinus infections.
Balloon sinuplasty is a relatively new, minimally invasive technique to open up the openings into the sinus cavities without the need for any tissue cutting or bony removal. When compared to the more traditional endoscopic sinus surgery, which does involve tissue cutting and removal, balloon sinuplasty causes less trauma to surrounding sinus and nasal tissues thereby:
  • Minimizing risk of CSF leak as well as damage to surrounding important structures including the eye, artery, optic nerve
  • Decreased pain
  • Faster recovery
  • Decreased risk of post-operative nosebleeds (thereby minimizing need for nasal packing)
  • Decreased chance of scar formation which may require repeated sinus surgery in the future
The balloon sinuplasty technique uses a balloon catheter system similar to that used in coronary artery angioplasty whereby a deflated balloon is inserted into the opening of the sinus cavity and inflated. When the balloon inflates, it opens up the sinus opening into a much larger diameter which than allows for drainage.

Sinusitis


Unfortunately, Millions of people suffer from sinusitis. However, we don’t have to suffer with this problem anymore.
Check out this article we found written by Dr. Christopher Chang from FAUquier ENT. In this article he explains a simple surgery, which allows your sinuses to function more effectively.

Simply put, sinusitis is an infection of the sinus cavities creating pus. Symptoms include facial pain/pressure, purulent drainage from the nose, fevers, etc. When the sinuses are functioning well, the infection gets cleared rapidly with antibiotics and the pus gets cleared easily through the nose. In this scenario, the openings of the sinus cavities into the nose are not blocked so things flow easily. No difficult concepts in this particular scenario.

Scenario #1 - Sinusitis IS Due to Infection
Scenario #2 - Pain/Pressure NOT Due to Infection
Scenario #3 – Tumors
Scenario #4 - Nasal Polyps
The goal of sinus surgery whether it is performed endoscopically or via balloon sinuplasty is to make the small drainage openings of the sinuses into the nose larger. That’s it! By making the sinus openings big enough, the sinuses can’t get obstructed as easily and therefore allow for good sinus drainage no matter how much swelling or inflammation may be present. Good sinus drainage prevents/minimizes sinus infections. Paradoxically however, sinus surgery may make your allergy symptoms worse (easier for allergens to gain access into your nose and sinuses since things are opened up), so it’s very important to get your allergies under good control before sinus surgery is performed. For those with asthma, having sinus surgery has been shown to markedly improve lung function resulting in decreased number of asthma attacks and use of inhalers overall.

What is Laparoscopic Adhesiolysis?

Laparoscopic adhesiolysis is a technique which is commonly used for the treatment of acute bowel obstructions. This procedure was earlier used by gynecologists for treating infertility and chronic pelvic pain. But today, it has become the treatment of choice for bowel obstructions as it has many benefits over invasive surgery. One of the biggest benefits of this procedure is that very less recovery time is required as the patient has to experience relatively less postoperative pain. The incidence of ventral hernia is drastically reduced and there are also fewer post operative complications. The return to normal bowel function is faster when compared to a surgery and the severity of intra abdominal adhesions after laparoscopic adhesiolysis is much less. People with complete or chronic bowel obstructions are usually advised to undergo this procedure.

What is Hysteroscopic Myomectomy?

Hysteroscopic myomectomy is a procedure that is employed for the removal of uterine fibroids. In this procedure, a hysteroscope is inserted into the uterus via the vagina and cervix. This instrument is quite versatile as it is very thin and hence no dilation is required to enter the cervix and into the uterus. A big advantage of this treatment procedure is that there is no need for any surgeries or abdominal incisions which drastically reduces recovery time for the patient. While a hysteroscope is used by the gynecologist to view the fibroid, the resectoscope is used to cut off the fibroid. The latter is basically a hysteroscope which is fitted at one end with a wire loop and makes use of high frequency electrical current to remove the fibroids.

Percutaneous Endoscopic Gastrostomy

What is PEG?
Percutaneous endoscopic gastrostomy or PEG is a procedure in which a tube is placed percutaneously inside your stomach aided by and endoscopic procedure. This is a very common procedure these days that is used for patients who are unable to ingest food on their own i.e. move food from the mouth to their stomach. Appendectomy

What Is Appendectomy?
An appendectomy is a surgery conducted in order to remove the vermiform appendix. This surgery is usually done when the appendix is inflamed or infected and if there is a danger that it will burst. This surgical procedure is done under general anesthesia because it involves making an incision in the lower right side wall of the abdomen. An appendectomy is a very important procedure because there is no other way of treating an infected or inflamed appendix. It is often done as an emergency procedure because the symptoms of appendicitis appear very quickly and also because a ruptured appendix is a life-threatening condition because of the large amount of infectious material that it releases into the bloodstream and abdomen. Appendicitis

What Is Appendicitis?
 Appendicitis is a painful and potentially dangerous condition caused by the inflammation of the vermiform appendix, a very small vestigial organ. The reasons for the inflammation vary from accumulation of thick mucus to the presence of hardened stool. The blockage of the appendix results in bacterial infection of the walls of the appendix. The most common symptom of appendicitis is acute abdominal pain that is very diffuse. The next stage is the inflammation of the abdominal lining called peritoneum. Other symptoms of appendicitis include nausea and vomiting. The inflamed appendix is in danger of rupturing, in which case the health of the patient is compromised. The only treatment for appendicitis is to surgically remove it via a surgery called appendectomy since it rarely responds to medication.

Gastric Endoscopy

What Is Gastric Endoscopy? 
Gastric endoscopy is a medical procedure whereby the doctor is able to view the inside lining of the patient’s digestive tract. This is a minimally invasive procedure that is performed with the aid of a highly flexible fiber optic tube that has a camera at the end that is inserted into the body. The other end of the tube has an eyepiece that enables the doctor to view the digestive tract easily or it can also be connected to a TV monitor. Since a gastric endoscopy does not require a large incision, it is generally done under local anesthesia. It is necessary because certain problems do not show up on conventional imaging reports. There have been instances of people suffering complications after a gastric endoscopy but these problems are rare.

Major inventions and improvements from Harold Hopkins


One of Harold’s major inventions was the zoom lens, which he created after a request from BBC. These new lenses opened up a new arena for television images and also paved the way for outside broadcasts and modern visual media. The second among his notable inventions is the development of fiber optics, fiberscope and endoscope. Fiber optics and fiberscopes have revolutionized the medical and manufacturing industry. A partnership between Hopkins and Karl Storz resulted in the invention of rod-lens endoscope, which provide an exceptional performance for endoscopy and paved the way for keyhole surgery. Hopkins was also able to bring about improvements to the laser disc and CD optical system by creating a mathematical analysis and calculated geometrical method, which played an immense role in bringing down the cost of CD players.

Who is Harold Hopkins?


Harold Horace Hopkins was a world-renowned British physicist whose ‘wave theory of aberrations’ revolutionized the field of optical design especially that of medical optics. This theory provides key mathematical analysis, which is used by modern computers to create high quality lenses. His notable inventions include zoom lenses, coherent fiber optics and the rod-lens endoscope, which paved the way for keyhole surgery, a technique that has helped millions of people worldwide. Hopkins has received many prestigious accolades and awards and has been nominated twice for Nobel Prize. In 2009, his son Kelvin Hopkins throws the Hopkins’s building to the public, to bring together both biomedical and pharmaceutical research bellow one roof of the University. Today, Hopkins stands for pursuit of excellence in both teaching and research.

Wolf and STORZ


Georg Wolf was a famous manufacturer of rigid endoscope and was based in Berlin; he established his business in 1906 and in 1911, developed the Sussmann flexible gastroscopes. Karl Storz set up business in 1945 and specialized in manufacturing endoscope for E.N.T. specialists. At that time, the technology used for making endoscope was quite primitive but Storz had a brilliant plan. He used bright and cold light, which, when introduced into the body cavity provided excellent visibility of the interiors and also allowed documentation of the cavity or organ with the help of image transmission. This technological advance by Storz, paved the way for modern endoscopy and a combination of his engineering vision along with advances made by Harold Hopkins, an optical engineer, revolutionized the field of medical optics.

History of endoscope and the person that developed it


An endoscope is a medical instrument that aids the doctor in the examination of the interior of any hollow organ or cavity of the human body. Endoscope can be directly inserted into the organ and consists of different parts like a rigid or flexible tube, optical fiber system, lens and eyepiece. Philipp Bozzini developed the first endoscope in the year 1806 but this invention did not gain popularity till 1822 when William Beaumont used the equipment on a human body. A major turning point in the use of endoscope was the use of electric light, which was available only for external use, but was soon developed for internal use like the invention of hysteroscope by Charles David and further experiments by Hans Christian Jacobaeus.

Endoscopes always need to be sterilized after a procedure

Endoscopic devices tend to collect a lot of microbes and debris that interfere with the functioning of the devices in addition to spreading diseases. As a result, they need to be cleaned with a great deal of care. It is very important for operators to follow the manufacturer’s instructions when using and cleaning this equipment in order to get it absolutely right. The device should be disassembled and cleaned very thoroughly in order to remove organic matter that inevitably gets attached to it. Care should be taken to avoid using abrasive materials when cleaning this device. It is also necessary to use a good enzymatic cleaner to break down proteins that have accumulated on the device. The devices also need to be sterilized, either with steam or chemicals, depending on the manufacturer’s instructions. All employees should receive training on how to clean endoscopic devices so that there is no possibility of anything going wrong. They should also be able to assemble the equipment well. The job of cleaning and sterilizing equipment should be given a great deal of importance so that patients undergoing the procedure do not come to any harm.

What is arthroscopic surgery used for?



Arthroscopic surgery is a very commonly requested procedure for people who have joint problems such as pain, swelling or instability.  This procedure basically involves looking inside a joint with the help of a device known as an arthroscope. This type of surgery is also called keyhole surgery because it requires a tiny incision in the skin that rarely exceeds 1 cm in length. This sort of operation is very easy to recover from and also has fewer side effects.
Arthroscopic surgery is usually done on knees and shoulders because they are large joints and therefore very easy to operate upon. Knees tend to have cartilage problems whereas shoulders usually have rotator cuff tears. This procedure is also used when the joint space is inflamed because small bits of bone or cartilage that have broken off into it. However, improvements in technology have ensured that this surgery is increasingly done on a wide range of joints such as elbows, hips, ankles and wrists which are usually very difficult to operate on.
